Teaching Licensure
Instructions for Addition of a Field to a Current MN Teaching License
Submit the following items to the Office of Records and Registration for institutional endorsement: (Any partial or incomplete submission will be returned to the applicant.)
Please submit:
- Completed application and conduct review statement
- Go to the Minnesota Department of Education (MDE) Web site at http://education.state.mn.us for license application.
- Click on Educator Excellence and then choose Licensing.
- Scroll down to Forms for Processes Not Yet Available Online and click on Application to Add a Field to an Existing Minnesota License. Download the forms.
- If problems, contact MDE at 651-582-8691.
- Complete the application:
- Complete Section 1 on p. 1 – Write in your file folder number.
- Complete Section 2 on p. 1 - Check the box for Addition of a New Field. Check the teaching box and write in the license for which you are applying. (Example: SPED-Learning Disabilities K-12)
- Complete Section 3 on p. 1.
- Complete Section 4 on p. 1 - Include ALL schools attended. If you did not earn a degree from a school leave degree code and year of degree blank.
- Section 5 on p.2 - Leave blank. This is SCSU’s recommendation section. Section 6 must be completed if you are applying for an administrative license.
- Complete the Conduct Review Statement on p. 3 and 4 - Fill in the identification information and answer all nine (9) questions. Be sure to sign and date page 4.
- p. 5 - Complete only if this applies to the applicant.
- A personal check, money order, or cashier’s check in the amount
of $62.70 made payable to the “Commissioner of MDE”.
- A $5 personal check, money order, or cashier’s check made payable
to St. Cloud State University. This is for your official SCSU
transcript. The licensing officer will request the transcript for you as soon as grades are recorded and verified.
- If you have attended other post-secondary institutions for this
license area, include an official transcript in the original
sealed envelope from each institution attended. The transcript(s) must
be sent to the applicant’s home. Do not open the envelope – keep it sealed! Transcripts from other institutions
must be included with your application for licensure. Do not have them
sent directly to SCSU. NOTE: Some institutions indicate that a transcript
sent to a student is unofficial. However, if the transcript remains in
an unopened, sealed envelope directly from the educational institution
that issued it, it is considered an official transcript for licensure
purposes.
- A copy of your current Minnesota License.
