Change health insurance carriers: BlueCross BlueShield, HealthPartners, PreferredOne

Add eligible dependents or drop dependents

StayWell Health Assessment

Optional & confidential

Obtain a $5.00 reduction in office visit copays by participating and agreeing to accept a health call

  • Dental

Enroll if you have waived or not enrolled previously

Select/change your dental carrier: State Dental Plan (Delta Dental) or HealthPartners State of Minnesota Dental Plan

Add or drop eligible dependents (children to age 26)

Waive Dental insurance entirely

  • Long Term Disability (LTD)

Enroll for LTD

Increase your coverage amount

Limited to approximately 60% of monthly income

  • Manager’s Income Protecting Plan (IPP)

Reduce the disability elimination period by one 30-day increment

  • Child Life insurance

Enroll if you have waived or not enrolled previously

Child Life – one premium provides a $10,000 policy for each eligible child (up to age 26)

  • Flexible Spending Accounts (FSAs) and Pre-Tax spending accounts

Benny Card replaces the Eide Bailly OneCard

New elections are required for 2014 plan year participation

Medical Dental Expense Account (MDEA) – Annual election: minimum $100, maximum $2,500

Dependent Care Expense Account (DCEA) – Annual election: minimum $100, maximum $5,000 family

The MDEA and DCEA can save participants roughly 25%-30% in taxes, but do have a Use it or Lose it rule

Transit Expense Accounts – For parking and transit expenses that are not payroll deducted
