Upon receipt of the request, Human Resources may need to meet with the employee to clarify their request and get additional information.
Human Resources will look at the current position description for the essential functions of the position. When necessary, Human Resources contacts the Supervisor for information on essential functions.
Human Resources sends an information form to the medical provider.
The medical provider completes the form and sends back to Human Resources.
After receiving information from the medical provider, Human Resources meets with the Supervisor that is most familiar with the work area to discuss the request. If it is determined that the employee is ADA qualified and the accommodation request is not an undue hardship, an accommodation will be discussed. The accommodation may not always be exactly what was requested. The funds to cover ADA accommodations need to come from the department of the employee or possibly a small account that Facillities Management has. If the accommodation that was requested is denied, the employee can appeal the decision.
To appeal, the employee must complete the ADA appeal form within 5 days of receiving the written response to the request. The appeal goes to the VP of Student Affairs, who has 10 days to make a final decision.
