Internship in Community Health

Student Reports and Daily Journal

1. Internship Confirmation Sheet is to be completed at the time of the agency interview, signed by the student and agency mentor, and returned to SCSU's Community Health Internship Coordinator for signature. The Confirmation Sheet must be completed at least four weeks prior to the start of the internship.

2. Orientation Report to be completed by the student in conjunction with the agency mentor. This report is due at the end of the first week of field experience.

3. Report (in the form of a daily journal) to be mailed, faxed, e-mailed to the SCSU Community Health Internship Coordinator on a weekly basis. Journals are to be received by the Internship Coordinator no later than Tuesday of the following week. Suggestions for journal:

   • Describe the program and your experience as you see it at the end of each day.

   • Describe how you are meeting your stated goals/objectives.

   • Keep a daily and weekly running total of hours worked.

   • Discuss what projects, activities, meetings and tasks you have been assigned.

   • Presentations made and to what groups.

   • Meeting professionals in the field associated with the internship.
