Graduate Assistantship Application
This application for employment as a graduate assistant should be turned into department of choice. The fourth page of this application lists departments and offices historically offering graduate assistantships. Individual Study Approval Form
This form is used to register for thesis 699 credits; creative
works 698 credits; or 501 courses. Also use this form
for internship credits; arranged courses; and special problems
600 credits.
Application for Graduation and Commencement
To be completed at the beginning of the semester in which the work for the degree is anticipated to be completed. Deadline dates for application for graduation are listed in the academic calendar.
Change of Graduate Program
For use when changing graduate programs or adding a second program to the existing program of study.
Graduate Course Permission Form
Undergraduates who wish to enroll in a graduate course must submit this form to the School of Graduate Studies.
Graduate Overload Petition
