Graduate Course Permission Form
For undergraduates who wish to enroll in a graduate courses (submit form to the School of Graduate Studies).
To seek permission to enroll in more than 15 credits per semester during the academic year or 15 credits during intersession, summer sessions I and II.
Individual Study Approval Form
To register for thesis 699 credits, creative works 698 credits, or 501 courses. Also used for internship credits, arranged courses, and special problems 600 credits.

Graduate Program Forms
Program-specific forms are for keeping track of your fulfilled program requirements. This must be submitted to the School of Graduate Studies after acceptance into a graduate program and before 16 credits have been completed. Meet with your advisor to ensure you are meeting your program requirements.
