Recommendation Form

Forms to send to colleagues and professors who will provide a reference for you. You will need three recommendation forms completed and returned directly to SCSU.

Affirmative Action Information

This form is an optional part of your application.

Special Student (non-degree seeking) Registration Form (online submission)

For students who are not admitted to a graduate program and wish to enroll in classes. This form will establish you in the SCSU registration system and allow you to register as a guest for graduate courses.

Financial Form

INTERNATIONAL STUDENTS ONLY
Financial information form to be completed with the application for international students.

Affidavit of Support

INTERNATIONAL STUDENTS ONLY
This form is to be used only if the international student has a U.S. sponsor.

Scholarship Application

Graduate scholarships are open to U.S. citizens and residents attending an on campus program. (The Shirley Schrader Scholarship is not available for fall 2011.)
