FormsApplication for Financial Aid (SCSU for 2009-2010): This one-page application provides additional information not included on the FAFSA and which must be taken into consideration to determine your eligibility for financial aid. It also provides a release of information if you, the student, wish to allow our office to visit with a third party (i.e., parent, spouse, etc.) about your financial aid. Bessie Campbell Short-Term Loan Application: A short-term loan for St. Cloud State University students who have unexpected, temporary educationally related expenses. Cancellation of Loan Funds: Students and parents may cancel or reduce federal, state, or private loan funds any time prior to disbursement or if the loan has already been disbursed, within 14 days after notification. To request a loan cancellation or reduction, please complete this form and return to the Financial Aid Office, Administrative Services Building Room 106, to make the request. Direct Deposit – Financial Aid: If your financial aid is in excess of the charges that you owe the University, the funds may be directly deposited into a bank account of your choice. Direct Deposit – Payroll: If you are employed through work study or regular student salaries, your earnings may be directly deposited into a bank account of your choice. Minnesota Achieve Scholarship: Awarded to Minnesota high school graduates who complete a rigorous program of study while in high school or a home-school setting. Minnesota GI Bill: This is an online application form through the Minnesota Office of Higher Education. Minnesota Indian Scholarship Application: Provides postsecondary financial assistance to eligible Minnesota resident students who are of one-fourth or more Indian ancestry and demonstrate financial need. St. Cloud State Scholarship Application – Returning Students: The Office of Scholarships and Financial Aid awards scholarships to a number of outstanding undergraduate students each year. Scholarship recipients are selected on the basis of their academic record, financial need and supporting information. This application is made available annually in early January. Work Study Application (pdf): You must have been awarded work study funding on your financial aid award letter. If you do not have work study funding as part of your financial aid, you do not need to complete this form. |
