Application for Financial Aid (St. Cloud State):This one-page application provides additional information not included on the FAFSA and which must be taken into consideration to determine your eligibility for financial aid. It also provides a release of information if you, the student, wish to allow our office to visit with a third party (i.e., parent, spouse, etc.) about your financial aid.

Budget Worksheet: A worksheet to assist you in determining your estimated education budget (costs minus financial aid).

Cancellation of Loan Funds: Students and parents may cancel or reduce federal, state, or private loan funds any time prior to disbursement or if the loan has already been disbursed, within 14 days after notification. To request a loan cancellation or reduction, please complete this form and return to the Business Services Office, Administrative Services Building Room 122, to make the request.

Consortium Agreement: A consortium agreement allows you to receive financial aid at St. Cloud State University for coursework taken at another institution (visited institution) if the coursework is applicable to your specific degree program of study at SCSU. This form is also available in the Office of Records and Registration, AS Building, Room 118. This form is used only if you will be attending another MnSCU school.

Evaluation Form - Student Employee: Supervisors are asked to complete a performance evaluation for their student employees minimally once per period of the work authorization and prior to the student’s termination of employment.  Evaluations may also be helpful midpoint during the employment period to provide the student employee feedback on positives and areas of growth.    

Minnesota GI Bill: The online application form through the Minnesota Office of Higher Education for eligible Minnesota veterans.

Minnesota Indian Scholarship Application: Provides postsecondary financial assistance to eligible Minnesota resident students who are one-fourth or more Indian ancestry and demonstrate financial need.

PLUS (Parent Loan for Undergraduate Students) Request Form: A federal loan program that enables parents to borrow funds to assist their son or daughter with educational related expenses. If you (the parent) are denied a PLUS loan, your student may be eligible for additional unsubsidized Direct Loan funds. Please contact our office if you have questions.

Release of Information: The Federal Education Rights and Privacy Act (FERPA) prohibits schools from providing non-directory information to anyone other than you (the student), including parents and family members, regardless of the age of the student or family support.  If you would like to give written permission for the Financial Aid Office to visit with someone other than you about your financial aid, complete and return this form to the Financial Aid Office. Scholarship Application (due March 15, 2012): The Financial Aid Office awards scholarships to a number of outstanding undergraduate students, who are selected on the basis of academic record, financial need, and supporting information. Awards generally range from $300 to $1,000. Applicants must have completed at least 15 credits at SCSU with priority given to those with a cumulative grade point average of 3.5 or better.
