Senior to Sophomore Course Verification

In order to ensure accuracy of class rosters, billing, and gathering of required data, please complete the following form by the dates listed on the Dates to Remember Calendar for each course in each semester or trimester.

All discrepancies have been reported to the S2S Program office and have been corrected.

I understand that any student who leaves the course for any reason must officially Drop or Withdraw from the class. Drop/Withdrawal forms must be sent to the S2S Program Office by the specified dates.
