Senior to Sophomore Course Verification

In order to ensure accuracy of class rosters, billing, and gathering of required data, please complete the following form by the dates listed on the Dates to Remember Calendar for all courses in each semester or trimester.

Responses are required for every item.








I have the signed "Student/Parent Acknowledgement" form on file for every student.



I have logged on to E-services and verified the class roster and it accurately reflects every student in the course.



I realize that if a student is not on the roster, they will not receive college credit for the course.



I realize that any student who leaves the course for any reason must officially Drop or Withdraw from the class. Drop/Withdrawal forms must be sent to the S2S Program Office by the specified dates.



I realize that if a student does not officially Drop or Withdraw from the course, they will receive an "F" for the course.
