Thank you for your interest in coming to the St. Cloud State Challenge Course. Here are the necessary forms that must be filled out by each individual prior to participating. You can either mail them in 7 days prior to your arrival or you can bring them when you come. Once a reservation has been made, we will need the following forms from you:

• Release Form (pdf)
• Medical / Health Disclosure Form (pdf)
• Needs Assessment Form (pdf)

* When possible all forms must be completed in advance of the program. It helps with the planning process and allows us to develop accommodations if necessary.

* All participants must complete these forms in order to participate.
