Challenge Course Required Forms
Thank you for your interest in coming to the St. Cloud State Challenge Course. Here are the necessary forms that must be filled out by each individual prior to participating. You can either mail them in 7 days prior to your arrival or you can bring them when you come. If you have any other questions, feel free to call Ivan Bartha at 320-308-6691.
Once a reservation has been made, we will need the following from you:
* All forms must be completed in advance of the program. If possible, it is very helpful to get them prior to the program to aid in the planning process.
