Program Report Template and Guidelines:
Use this template if you are reporting the assessment activities of a major program, graduate program, certificate program, and free-standing minor program (ones that don’t have a corresponding major program).
General Education Report Template and Guidelines
Use this template if you are reporting assessment activity for the 2008-9 academic year for General Education. This form should be submitted only by departments that have assessed one or more General Education courses in the past year. If your department has not conducted General Education assessment, do not submit the form.
Upper Division Writing Requirement (UDWR) Template and Guidelines
Use this template if you are reporting the assessment of UDWR in your program for the 2008-9 academic year. This form should be submitted by departments that have assessed UDWR courses in the past year. If your department has not conducted UDWR assessment, do not submit the form.
